<p dir="ltr">A 75-year-old female presented with type A acute aortic dissection, with computed tomography showing concomitant ostial left main stem (LMS) compression by an extra-coronary haematoma. This corresponded to anterior ST-segment elevation and severe antero-septal wall hypokinesis. Before emergency surgery, coronary angiography (CAG) and intravascular ultrasound (IVUS) confirmed occlusive luminal narrowing of the ostial LMS to the proximal left anterior descending artery (LAD) due to extending extra-coronary haematoma (<i>Panels A and B</i>; arrows indicate lumen compression).</p>
